Catholic Charities San Bernardino & Riverside Counties is deeply committed to ensuring that all individuals and families have access to quality healthcare, regardless of their financial situation. Our Healthcare Access and Support program provides comprehensive services to help clients navigate the complex healthcare system, secure necessary medical coverage, and receive the care they need. This program includes Medi-Cal enrollment and redetermination assistance, medical guidance and support, telehealth services through our Health Hub, and Primary Enhanced Provider (ECM) services. Additionally, our efforts are rooted in a commitment to addressing the social determinants of health—recognizing that factors such as housing, education, employment, and access to nutritious food play a critical role in the well-being of our communities. Our goal is to improve health outcomes by connecting our community members to vital healthcare resources and services while also addressing the broader factors that impact their health.

Qualifications and Eligibility

Eligibility Requirements
To qualify for CalFresh, applicants typically need to meet the following criteria:

    • Residency: Must reside in California.
    • Income Limits: Household income must fall below certain limits based on family size and circumstances (e.g., seniors, persons with disabilities).
    • Citizenship or Qualified Immigration Status: U.S. citizens and certain legal residents may qualify.
    • Other Criteria: Able-bodied adults without dependents may have additional work requirements.
